An Apotek holds one critical privilege no other retail type has: it can legally dispense prescription drugs (obat keras, obat etis) and prepare galenical compounded medications. The hard line: if you want to sell any medication that requires a doctor's prescription, you are an Apotek (47721), not a Toko Obat (47722). This distinction is enforced — operating without the correct license category exposes you to Kemenkes sanctions and criminal liability under UU Kesehatan.

The APA requirement — the critical constraint
The single biggest practical barrier to opening an Apotek is the Apoteker Pengelola Apotek (APA) — the responsible pharmacist who must:
- Hold a valid STRA (Surat Tanda Registrasi Apoteker — national pharmacist registration)
- Hold a valid SIPA (Surat Izin Praktek Apoteker — local practice license, issued by Dinas Kesehatan Kabupaten/Kota)
- Be physically present during pharmacy operating hours (or have a substitute Apoteker Pendamping during their absence)
The SIPA is specific to location — an Apoteker licensed for Denpasar cannot serve as APA for a pharmacy in Badung without separate approval. Without a confirmed APA with valid SIPA, your SS application will not proceed.
